Passport

Passport funding, through the Ministry of Children, Community and Social Services, gives adults with a developmental disability an opportunity to take part in their community. Those who receive Passport funding have the opportunity to pursue what they enjoy, their personal goals, and what they want to achieve, as well as respite. People may choose to develop work skills, volunteering, and being involved further in their community, building skills to help in their everyday life.

Sound like a lot? Here’s how Extend-A-Family Waterloo Region can help!
  • We will build an individualized plan with you, for your funding, based on your wants and needs
  • We will always use an approach that puts what you want first
  • We will explore choices and options including one-to-one support, community involvement programs, or a combination of both
  • We will develop a budget
  • We can directly manage your funding
  • We can manage reimbursement to Direct Support People and third-party services
  • We can pay bills directly to services and supports covered by your funding
  • We will make sure that Ministry documentation and requirements are met

Our experienced Support Coordinators can help you access all that your Passport funding can do. Not only can we help connect you with supports in the community, Extend-A-Family Waterloo Region also has a large network of Direct Support People to help you achieve your goals and dreams.

Things to consider before connecting with our Passport Support Coordinators:
What type of support do you need?

Daytime, evening, weekend? In the home? In the community?

What type of support do you want?

My own Direct Support Person? Flexible hours or a routine schedule? Be a part of a group in the community?

What are you interested in?

Getting a job? Discovering interests and hobbies? Getting out into the community?

We are the first agency in the Waterloo Region who can act as a direct brokerage for your funding, saving you time and money from your funding.

  • Open Space

    Open Space is an in-community gathering for people of all abilities. Join in and enjoy good food, conversation, and fun! 

  • Supported Extended Experiences and Discoveries

    Interested in using your Passport funding to explore Ontario and try something new? SEEDS is for you!

  • Weekend Social Club

    Take part in recreational activities on a monthly basis, from sporting events and nature hiking, to visiting historical places!
